FQ9.乳癌術後のサーベイランスとして血清腫瘍マーカー測定は推奨されるか?

3.乳癌の精密検査(治療後)

ステートメント
・乳癌術後のサーベイランスとしての血清腫瘍マーカー測定による生存率,QOLの改善を示す結果はいまだ得られていない。

背 景

本学会また海外のガイドライン等でも術後のサーベイランスとして血清腫瘍マーカーの測定は推奨されていない1)~5)。しかし,実臨床では広く使われているのが現状である6)~8)ため,今回フューチャーリサーチクエスチョン(FQ)として,「乳癌術後のサーベイランスとして血清腫瘍マーカーの測定は推奨されるか?」を取り上げ,現時点のエビデンス,現在進行中の臨床研究の検索を行った。

解 説

本ガイドライン2015年版(検診・画像診断)では,「総論:初期治療後フォローアップ」としての腫瘍マーカーの測定について,次のように記載されている。「乳癌初期治療後の腫瘍マーカーとして,CEA,CA15-3を測定することが多い。再発をきたした場合,CA15-3は上昇し9)~12),症状出現もしくはその他の検査で発見される平均5~6カ月前に,CA15-3の上昇によって再発を予測することができる3)。しかしながら,腫瘍マーカーの結果により,無症状の潜伏転移を治療することの意義は証明されていない」。また,推奨グレードに関しての記載はない。

今回,乳癌術後のサーベイランスとして血清腫瘍マーカーの測定研究に関して,下記に記載したキーワードを用いて,検索を行った。診断性能,つまり上昇時の再発の感度および特異度,利益として生存率,QOLの改善,不利益として放射線被曝,精神的負担,費用に関しての検討を試みた。

これまで,術後サーベイランスとしての腫瘍マーカーの診断性能に関するいくつかの前向き研究が行われている12)~15)。各研究での感度はCEA 34.4~39.8%,CA15-3 35.6~70.2%,CEAとCA15-3併用で51.6~72.6%,また特異度はCEA 91.2~100%,CA15-3 95.7~100%,CEAとCA15-3併用で88.3~100%であった。いずれの研究でも,遠隔転移と比較すると局所領域再発についての感度は低かった12)~15)。ただ,これらの研究のカットオフ値は異なっている。また,今から20年以上前の研究であり,再発の確定診断は,胸部単純撮影,腹部超音波検査,骨シンチグラフィなどで行われていた。

乳癌術後のサーベイランスとしての血清腫瘍マーカー測定に関する比較的新しい研究として以下のようなものがある。Di Gioiaら16)は,813例のフォローアップ症例を用い,前向き研究を行っている。腫瘍マーカーとしてCEA,CA15-3,CA125を6週ごとに測定したところ,観察期間中央値63カ月内に,44例で上昇を認め,全身MRI,FDG-PET/CTの両方あるいはどちらかを施行した。そのうち29例(65.9%)で遠隔転移を発見,1例(2.3%)で局所再発,6例(13.6%)で二次癌の発見を認め,遠隔転移のあった29例のうち7例(24.1%)は1臓器に限局する4個未満の限局した病変であった。また9例(20.5%)では悪性所見なく,特異度98.9%であった。また再発例の無増悪生存期間の中央値は9.2カ月,全生存期間の中央値は41.1カ月と報告されている。本研究ではコントロール群の設定がなく,生存率の改善は示せていない。

今までの研究で,腫瘍マーカーの感度,特異度は比較的良好な値であり,再発の診断に有用であることが示されてきているが,生存率,QOL,精神的負担,費用の観点において,現在のところ有効性を示す研究は存在しない。しかし,無症状の乳癌患者に腫瘍マーカーをモニタリングすることの論拠は乳癌治療において理論上,腫瘍量が増加する前の段階の腫瘍量が少ない時期に治療を開始したほうが治療効果を期待できるからである17)。一方,補助療法は検出できない微小転移を取り除き,遠隔再発リスクを減らし,全生存率を改善する18)。再発乳癌を早期の治療で予後を改善することは統計学的な有意なデータはないが,早期で限局した再発は治癒する可能性がある19)20)

このような背景において,2つの進行中もしくは計画中のランダム化比較試験が認められた。1件目は現在進行中の臨床研究でThree-monthly Dynamic Evaluation of CEA and CA 15.3 vs Usual Practice in the Follow-up of Early Breast Cancer Patients:a Randomized Study(NCT02261389)21)である。StageⅠ~Ⅲ乳癌術後患者800人を対象として,現在対象者を募集中である。この試験の目的は,無症候性患者において血清CEAおよびCA 15―3を3カ月ごとに測定し,明らかな増加した場合のみ行われる画像診断(18FDG-PET)は,通常の日常臨床のフォローアップと比較して,局所領域再発(乳房内は除く)および遠隔再発の診断を予測することができるかの検討である。主要なアウトカムとしてランダム化してから遠隔再発の診断までの期間を設定している。二次的なアウトカムとしては,CEAおよびCA15-3の陽性適中度(PPV)および陰性適中度(NPV),サブタイプ(ERおよびHER2発現状況)における診断予測についての探索的分析,対照群と比較して,介入群で回避される画像診断検査の回数,またQOLを比較することを予定している。

また2件目は,SWOGおよびNCIの臨床試験ネットワークによってランダム化比較試験が計画されている。StageⅡ~Ⅲ乳癌患者の腫瘍マーカーの連続測定を行い,これにより早期治療開始が全生存率を改善するかを評価するものである22)。現在の試験デザインでは,3~6カ月ごとに血清腫瘍マーカーを5年間検査するが,患者および医師は検査結果を知らされない。フォローアップ中に腫瘍マーカーが上昇した場合,医師に結果を報告する(非盲検化群)か,報告しない(盲検化群)かの,いずれかにランダムに割り当てられる。非盲検群では,マーカー上昇が確認された患者は再発の確認後,医師および患者は治療を開始または変更することが強く推奨される。盲検群に割り付けられた患者は,通常の臨床フォローアップを継続し,再発が臨床的に明らかになった時点でその評価と治療を受けることになる。

今後,これらの臨床研究で,乳癌術後のサーベイランスとしての血清腫瘍マーカー測定が生存率,QOLの改善に有用であると証明されれば,今後の日常臨床に推奨されることになる。
