BQ1.非浸潤性乳管癌に対して乳房温存療法は勧められるか?

1.乳癌初期治療における乳房手術

ステートメント
・非浸潤性乳管癌に対する乳房温存療法は,浸潤癌における乳房温存療法の適応に準じて行う。

背 景

非浸潤性乳管癌(DCIS)は乳房全切除術により根治が得られると考えられるが,現在では術後放射線療法を併用した乳房温存療法も広く行われている。DCISに対する乳房温存療法について解説する。

解 説

DCISに対する乳房温存療法に関しては,レビューが多く存在し,DCISに対しても乳房温存療法は,浸潤癌と同様に,組織学的に断端陰性で整容性が保たれるならば,標準治療と考えられている1)~4)
乳房温存療法の場合には,局所再発のリスク因子が推定されている。PangらによるDCISに対するリスク評価の論文のレビューによると,臨床所見としては若年(40歳未満),マンモグラフィ高濃度乳房,有症状,多発病変,病理学的所見としては,高核グレード,面疱壊死,切除断端露出または近接,バイオマーカーとしてはER陰性,HER2過剰発現,細胞周期マーカー(Ki67)などが再発のリスク因子として挙げられている5)

一方で,低リスクの患者を選別するためのprognostic index(予後指数)も開発されている。そのなかで多く用いられているのがVan Nuys prognostic index(VNPI)で,腫瘍径,断端距離,病理学的分類(核グレード,面疱壊死),年齢よりスコアを算出し,低ければ部分切除のみの,高ければ乳房全切除術の,中間であれば乳房温存手術+術後放射線療法の適応としている6)。Rudloffらは,メモリアルスローンケタリングがんセンター(MSKCC)において,これら病理学的因子以外に,家族歴,年齢,発見契機など10項目から,局所再発率を計算するMSKCCのノモグラムを作成し,報告している7)

術後放射線療法については,ランダム化比較試験のメタアナリシスにおいて局所再発が約半分(28%→13%)に減少することが報告されている(放射線BQ2参照)。一方で,術後照射が省略可能な予後良好群を同定する試みもなされており,RTOG 9804試験の報告では腫瘍径2.5 cm以下の低グレードから中間グレードのDCIS症例で断端距離が3 mm以上の場合には,術後放射線療法と放射線療法省略群の7年の局所再発率は0.9% vs 6.7%と非照射群でも低く,無増悪生存率や全生存率には有意差は認めなかった8)。また,SagaraらよるSEERデータを用いた後ろ向きコホート研究によると年齢・腫瘍径・核グレードからなる予後スコアと術後照射による予後の改善効果は関連しており,術後照射を省略可能な症例を選別できる可能性を報告している9)。また,多遺伝子アッセイ(Oncotype DX DCIS)を用いた術後照射省略の可能性も報告されている10)

以上より,DCISに対する乳房温存療法は,乳房全切除術と並んで,確立された治療法と考えられる。将来的には一部の低リスクDCISに対しての術後照射省略可能な乳房温存手術や非手術など,さらなる治療選択肢ができる可能性があり,今後の臨床試験の結果が待たれる11)

検索キーワード・参考にした二次資料

「乳癌診療ガイドライン①治療編2015年版」の同クエスチョンの参考文献に加え,PubMedで“Carcinoma,Intraductal,Noninfiltrating”,“Mastectomy,Segmental”のキーワードで検索した。検索期間は2014年1月から2016年11月までとし,180件がヒットした。また,UpToDate 2017,The Cochrane Library 2014を参考にし,これらの文献のなかから必要なものを抽出した。
