2.転移・再発乳癌

ステートメント
・ER陽性HER2陰性転移・再発乳癌に対する一次治療は,基本的に内分泌療法を行う。
・急速な病状進行や高度の全身転移など命を脅かす状況,また内分泌療法耐性と考えられる症例では化学療法を行う。

背 景

転移・再発乳癌治療の主な目的は生存期間の延長とQOLの改善であり,その薬剤選択はHortobagyiが提唱したアルゴリズム1)や各種ガイドライン2)3)をもとに行われている。ER陽性HER2陰性転移・再発乳癌における再発後の予後因子として,再発までの無病生存期間,再発部位(内臓転移の状況)や内分泌療法感受性などがあり,これらをもとに一次治療が選択されている。ここでは,一次治療として,内分泌療法と化学療法のどちらを行うべきかを概説する。

解 説

ER陽性HER2陰性転移・再発乳癌に対する一次治療は,NCCNガイドラインではvisceral crisisの状態であれば化学療法から開始し,それ以外は内分泌療法を推奨している。ASCOガイドラインでは,生命を脅かす状態(life threatening disease)や内分泌療法耐性以外では内分泌療法を推奨している2)。また,ABC3コンセンサスガイドラインでもほぼ同様の推奨となっている3)。転移・再発乳癌に対する一次治療として内分泌療法と化学療法を比較したランダム化試験を,全生存期間(OS)に関して5試験,全奏効割合(ORR)に関して6試験のメタアナリシスを行った4)~9)。その結果,OSに差は認められず(HR 0.95,95%CI 0.80-1.13,p=0.58),ORRも同等であった(HR 0.76,95%CI 0.48-1.21,p=0.25)。副作用は3試験で評価されており,嘔気,嘔吐,脱毛がいずれも化学療法群で有意に高い結果であった。QOLに関するデータは不足しているものの,益と害のバランスから内分泌療法が化学療法より優れており,基本的には内分泌療法を行うことが勧められる。一方で,両者を比較したランダム化試験はいずれも古い年代のものであり,現在の治療体系に合致しているとはいえない。急速な病状進行や高度の全身転移など命を脅かす状況では,速やかな腫瘍量の制御を目的とした化学療法が推奨される。また,内分泌療法耐性と考えられる症例においても化学療法の適応と考えられる。

Breast Cancer. ver. 3. 2017. を参考にした。
