BQ13.乳癌治療として補完・代替療法は勧められるか?

ステートメント
・乳癌の進行抑制や延命効果の目的で補完・代替療法は行うべきでない。
・標準的な癌治療に伴う症状の緩和や不安の軽減などを目的とした補完・代替療法には検討の余地がある。

背 景

癌患者における補完・代替療法(complementary and alternative medicine;CAM)は,文献検索レベルで海外の乳癌患者では平均45%(17~75%)1),国内癌患者では44.6%の方が行っている。乳癌や肺癌,肝臓癌では他部位よりも高率2)と報告され,多くの乳癌患者が使用しているが,それを医師に伝えているのは半数に過ぎないとの報告がある3)。CAMの内容は人種間で違いがみられ3),わが国のCAM利用者の9割以上が漢方やアガリクスなどのキノコ関連,サメの軟骨やビタミンなどの製品を使用している2)。欧米患者の利用目的は癌の進行に伴う痛みなどの症状緩和や心理的不安の軽減,通常の癌治療に伴う有害事象の症状緩和などである一方で,わが国では3分の2が癌の進行抑制を期待してCAMを使用している2)。国内CAM利用者の半数以上は十分な情報収集や専門医師への相談を行わずに,広告やインターネットなどの情報,友人や家族からの勧めをもとに利用している状況である2)。わが国の癌治療医の多数はCAMに対して否定的な見解をもっているが4),患者は自身の健康や人生に関する価値観や信念に合致することからCAMを受けているとの報告5)もあり,CAMを頭ごなしに否定するのではなく,このような治療を選択する患者の心理に理解を示すことも重要である。

解 説

2004年に設立された米国統合腫瘍学会(Society for Integrative Oncology;SIO)が,1990~2013年に報告された乳癌とCAMに関する4,900論文から203編のランダム化比較試験をレビューして2014年にガイドラインを発表し,2017年には米国がん協会によりその改訂版が発表された6)。この中で乳癌治療中,治療後におけるエビデンスレベルの高い補完・代替医療が表1のように示された。

ランダム化比較試験より容認されるCAMとしては,肥満乳癌患者に対する脂肪摂取制限7)8),内分泌療法によるホットフラッシュに対する鍼治療9),ホットフラッシュによる睡眠障害に対する電気鍼治療10),痛みや不安に対するマッサージ療法,適度な運動,サポートグループ11)やリラクセーションなどによる心理療法や心体介入などが挙げられる。

日本人を対象とした報告12)13)からは,心理療法や心理サポートが乳癌患者の症状や感情の改善に役立つことが示唆されている。

これまでCAMによる抗腫瘍効果,再発抑制,延命に関する十分な証拠は得られておらず,上記ガイドライン6)でもこの点に関する推奨はされていない。

アガリクス摂取による劇症肝炎14)のように,CAMには報告されていない重篤な有害事象が起こる可能性があり,安易に利用を勧めることがあってはならない。また,標準的な治療を行わずにCAMのみを選択することは病状の進行や生命予後の短縮に結び付く可能性があり15)~17),科学的根拠に基づいてその効果と安全性をしっかりと患者や家族に説明する必要がある。

CAMに関してはSIO18),米国国立補完統合衛生センター(National Center for Complementary and Integrative Health;NCCIH)19)のホームページに詳細な情報が掲載されており,患者への情報提供に有用である。わが国では厚生労働省「統合医療」に係る情報発信等推進事業「統合医療」情報発信サイト20)に,統合医療に関するさまざまなエビデンスが掲載されている。

検索キーワード・参考にした二次資料

PubMedで“Breast Neoplasms”,“Aromatherapy”,“Massage”,“Relaxation”,“Psychotherapy”,“Complementary Therapies”のキーワードで検索した。検索期間は2016年11月までとし,159件がヒットした。また,UpToDateの“Complementary and alternative therapies for cancer”(Last updated:Aug. 15, 2016)を参考にした。
