2.乳癌発症リスク―①生活習慣・環境因子

ステートメント
・緑茶の摂取が乳癌発症リスクを減少させるかどうかは結論付けられない。

背 景

緑茶は日本およびアジアの一部で習慣的に飲用されているが,アジア地域では乳癌の発症率が低いことが知られている。また,基礎的研究では,緑茶の主成分であるカテキンに乳癌細胞の増殖を抑制する効果があることが示されている(二次資料①)。さらに,緑茶のカテキンにより前立腺癌の発症が抑制されたとの報告もみられる(二次資料②)。この項では「緑茶の習慣的な摂取と乳癌発症リスク」の間に関連性があるか否かについて概説する。

解 説

本ガイドライン2013年版における文献検索の結果,3件のコホート研究1)~3),4件の症例対照研究4)~7),3件のメタアナリシスやシステマティック・レビュー8)~11)が抽出された。コホート研究では,いずれも緑茶摂取による乳癌のリスク減少は認められなかった。一方,症例対照研究では,いずれも有意のリスク減少が認められた(2件において代謝酵素遺伝子の多型と緑茶の効果との関連が検討された)5)6)。また,メタアナリシスでは,1件で有意なリスク減少が認められたが9),残りの3件では認められなかった8)10)11)。しかし,そのうち1件では緑茶摂取量の増加に伴い乳癌再発リスクが減少することが示されていた11)。日本人の大規模コホート研究では,緑茶摂取によって上昇する血清緑茶ポリフェノールと乳癌発症の関連は否定されていた12)

本ガイドライン2015年版の論文検索では日本人を対象とした症例対照研究1件13)が報告されているが,関連なしとされていた〔OR 1.27(95%CI 0.75-2.14,傾向検定のp=0.20)〕。

2007年にWCRFとAICRが作成した「Food,Nutrition,Physical Activity and the Prevention of Cancer:a global perspective」第2版では,「緑茶摂取と乳癌発症リスク」の関連に関し信頼性の高い研究結果は存在しないと評価された(二次資料③)。2017年には更新版が公開されたが,内容に変更はない(二次資料④)。

これまでの研究結果にばらつきがあり,近年の信頼性の高い論文はその関連を否定するものが多く,日本人の研究に限ってもそれは同様である。以上より,緑茶の摂取が乳癌発症リスクを減少させるかどうかは,結論付けられないと判断した。

検索キーワード

PubMedで“breast neoplasms”,“Tea”,“Camellia sinensis”,“Risk”のキーワードを用いて検索した。検索期間は2014年12月~2017年5月としたが,本BQの趣旨に合致する新たな文献はなく,2015年版(CQ5)と同様の14文献を採択した。また二次資料として1件を追加した。
