BQ7.転移・再発乳癌に対して動注化学療法は勧められるか?

2.転移・再発乳癌

ステートメント
・転移・再発乳癌には全身薬物療法が標準治療であり,日常臨床として動注化学療法は行うべきでない。

背 景

転移・再発乳癌治療の目的は生存期間の延長とQOLの改善であり全身薬物療法が標準治療である1)が,局所制御目的に局所進行乳癌および肝転移に対して動注化学療法が試みられている。

解 説

1)局所進行乳癌における動注療法

切除不能な局所進行乳癌に対して1980年代より局所動注化学療法が試みられてきた。局所進行乳癌に対する動注化学療法と全身化学療法を比較した唯一のランダム化比較試験の結果2)では,エピルビシン50 mg/bodyをday 1,4,7(計150 mg/body)投与した化学療法を行った場合,動注化学療法は全身化学療法をしのぐ局所制御率を示したと報告された。しかしながら,動注化学療法は全身化学療法と比較して無病生存期間や全生存期間を改善せず,また,この試験で用いられた全身化学療法薬の総投与量(エピルビシン 計150 mg/body)が標準的全身化学療法よりも相当少なく,この試験の結果の解釈には注意を要する。また,局所動注化学療法の症例集積の報告3)~6)によると,局所進行乳癌に対する動注化学療法の奏効率は50~80%前後であり,一方,局所進行乳癌を含む患者を対象とした術前化学療法の臨床試験(NSABP B-27試験,n=2,441)の結果は,AC療法(各60,600 mg/m2 3週毎)4サイクルによる全身化学療法で奏効率85.5%,順次的にドセタキセル(100 mg/m2 3週毎)4サイクル追加投与した場合の奏効率は90.7%と報告されている7)。したがって,間接的な奏効率の比較ではあるが,動注化学療法が全身化学療法に勝るというエビデンスは存在しない。

2)乳癌肝転移における動注療法

乳癌肝転移に対する動注化学療法(肝動注)に関しては,肝動注の第Ⅰ/Ⅱ相試験8),症例集積9)~11)などの報告が存在し,奏効率は40~60%前後である。また,MEDLINEなどのデータベースをもとに,2000年から2012年までに発表された主な転移臓器が肝臓である乳癌患者(n=8,260)を対象にした臨床研究をまとめた報告がある12)。その結果から,全身化学療法の臨床試験(合計:27試験,n=4,958)では生存期間の中央値が17.9カ月(範囲6~29.2カ月)であったのに対し,動注化学療法の臨床試験(合計:6試験,n=373)では,生存期間の中央値が27.9カ月(範囲18.5~47カ月)であった。ただし,この報告では患者背景や解析方法が不明であることや,全身化学療法と動注化学療法のランダム化比較試験が存在しないため,得られた結果を高いレベルのエビデンスと解釈することはできない。また,動注化学療法に特有の合併症として留置カテーテルトラブルが2~4割に認められること,臨床試験の数が少なく肝動注の最適なレジメンが不明なこと,全身化学療法に比べて手技が煩雑であることなどが肝動注治療の問題点である。

以上より,局所進行乳癌および乳癌肝転移に対して動注化学療法が全身化学療法に比較して勝っているという前向き研究からのエビデンスはなく,症例報告の統合解析に関しても出版バイアスの存在を否定できず,生存期間に与える影響は不明である。最適レジメンが不明であり,手技が煩雑でカテーテルトラブルのリスクがあることなどを考慮すると日常臨床で転移・再発乳癌に対する局所薬物療法は行うべきでない。
