BQ4.ホルモン受容体検査はどのような目的で,どのように行うか?

ステートメント
・内分泌療法の適応があるか否かを知るために,ホルモン受容体の発現状況を検索する。検査には免疫組織化学法を用いる。

背 景

内分泌療法は乳癌薬物療法における主要な柱の一つであるが,内分泌療法の適応があるか否かは,癌組織におけるホルモン受容体の発現状況により決定される。ホルモン受容体の臨床的意義と,その検査法の実際,最近の動向について概説する。

解 説

1)ER,PgR検索の臨床的意義

内分泌療法は,ER陽性浸潤性乳癌症例に再発・死亡抑制効果があるが,ER陰性症例にはほぼ無効であることが,数多くのランダム化比較試験,および大規模なメタアナリシスで示されている1)~3)。ERの内分泌療法効果予測因子としての有用性は,患者背景,内分泌療法の種類・期間,化学療法の有無,癌の進行状況によらず確認されており,ER検索は一貫して乳癌診療上の必須項目とされている。

PgRは,エストロゲンによりERを介し誘導されるERの標的物質の一つであり,PgR発現の有無はエストロゲンとERの機能が正常に働いているかの目安になるとされている。臨床的にもER陽性PgR陽性癌はER陽性PgR陰性癌よりも,ER陰性PgR陽性癌はER陰性PgR陰性癌よりも,内分泌療法に対する応答性が高い可能性があることが示されている2)4)5)。そのためPgRもERと同様に検索されるようになったが,2010年前後,その検索意義について相次いで疑問が呈された2)6)。一方,PgRの内分泌療法効果予測因子・予後予測因子としての有用性については引き続き多くの報告がなされており4)7)8),PgR検索は今日も乳癌診療上必須の項目となっている。

非浸潤性乳管癌(ductal carcinoma in situ;DCIS)は,局所のみの治療(手術±放射線治療)で予後が極めて良好な乳癌だが,DCISに対する内分泌療法は,乳房温存療法後の局所再発予防の目的で考慮されることがあり(乳癌診療ガイドライン①治療編2018年版,薬物CQ5参照),この場合にはDCISのホルモン受容体発現検索が行われる。

転移・再発乳癌,手術を行っていない進行乳癌においても,乳癌組織におけるホルモン受容体の発現状況は,内分泌療法の効果予測,あるいは再発後の予後予測のうえで重要であることが,多くの研究で報告されており,乳癌組織(原発巣,転移巣いずれか少なくとも一方)におけるホルモン受容体検索が強く勧められる9)。また,再発・転移組織と原発巣組織で,ホルモン受容体の発現状況が一致しない症例が少なからず存在することが報告されており,再発・転移巣組織が入手可能な場合には,ホルモン受容体発現状況を再度,検索することが勧められる10)11)

したがって,すべての原発乳癌において,内分泌療法の適応を決定するためにホルモン受容体の発現状況を検索することが強く勧められる。原発巣の評価に際しては,浸潤巣,非浸潤巣を区別することなく検索するのが一般的だが,両者に明らかな差がある場合には,その旨を付記することが望ましい。

2)ホルモン受容体の検索方法とカットオフ値

ホルモン受容体発現状況は,2000年頃までは乳癌凍結組織を用いた生化学的方法(dextran coated charcoal法やEIA法)で調べられ,前述の臨床試験の多くも生化学的方法によるものだった。後に,免疫組織化学法(IHC法)による検索が可能となり,IHC法でも生化学的方法と同等以上に,優れた結果が得られることが示された12)。IHC法は生化学的方法に比し多くの利点があり(一般の病理検査室で実施可能,種々の材料で検索可能,陽性細胞がごく少数でも検出可能,永久標本についての後方視的な観察が可能等),現在,IHC法による検索が標準となっている。

実臨床で通常用いられるホルマリン固定パラフィン包埋(formalin fixed paraffin embedded;FFPE)検体を用いた検索では,検体の取り扱い方法が結果を大きく作用するため,厳しい精度管理が求められる(病理:総説2参照)。

染色にあたって,日本乳癌学会の「適切なホルモンレセプター検索に関する研究班」は,体外診断用医薬品として市販されている一次抗体を使用し,製造販売元のプロトコールに従うことを推奨している13)染色スライドには,乳癌とともに内因性コントロールである非癌の乳管・小葉上皮が含まれていることが望ましい。また,検査の際には,外因性コントロールスライドも同時に染色すべきである14)

判定の際には,まず,HE染色標本,外因性コントロール,内因性コントロールを検鏡し,技術的な問題の有無を確認する。技術的な問題が疑われる場合には,当該標本で判定せず,再染色を行う13)14)。判定方法は,陽性細胞の占有率で判定する方法と,占有率と染色強度を組み合わせる方法に分類されるが,内分泌療法の効果予測性という点において,判定方法による明らかな差は報告されていない15)16)。一方,陽性細胞の占有率で判定する方法が,占有率と染色強度を組み合わせる方法よりも,染色方法間や観察者間の診断一致率が高かったとの報告もある17)。このため日本乳癌学会研究班では,実際的で再現性のある判定方法として,陽性細胞の占有率で判定するJ―Scoreを推奨したが,染色強度を加味した判定方法との併用を妨げるものではないとしている13)。ASCO/CAPガイドラインでも,判定の際には陽性細胞の占有率を用いるが,陽性細胞の占有率とともに平均染色強度も報告することが推奨されている14)。UK NEQASでは,簡単で有用性が確認されている方法として,陽性細胞の占有率と染色強度を組み合わせるAllred scoreが紹介されている12)

陽性細胞の占有率で判定する方法における陽性/陰性のカットオフには,any positive cell(陽性細胞が1個でもある),1%,10%などがある。10%は,すでに臨床的意義が確かめられていた生化学的方法による陰陽判定との一致性を根拠としており13),以前はカットオフ値として最も一般的に用いられていた。1%は,IHC法データに基づく臨床研究からAllredらが提唱したAllred score>2にほぼ相当する9)12)。内分泌療法は予後改善に有用で重篤な副作用も稀なため,内分泌療法の適応範囲を広くする意味合いもあり,ASCO/CAPガイドラインでは1%が推奨され,現在広く用いられている。しかし,「ホルモン受容体陽性症例」の中でも,陽性細胞占有率が高い症例と低い症例では,内分泌療法奏効性が異なるとの多くの報告がある5)16)18)。2017年のザンクトガレンコンセンサス会議では,「ERかつ/またはPgR陽性細胞率1%以上」を「ホルモン受容体陽性」としつつも,ホルモン受容体発現程度により,その他の臨床病理学的諸因子も考慮しての,異なる治療の推奨(内分泌療法の種類や化学療法の追加など)がなされている19)。2015年の同会議では,ER陽性率1~9%をequivocalとし,10%以上と区別する推奨がなされていたが19)20),2017年では「高発現」「低発現」についての具体的数値の提示はなされていない19)。また,「1%」と「any positive cell」が同一視されていることも稀でなく,両者を区別する意義を疑問視する臨床研究もある5)

これらを総合的に考えると,ER,PgRとも陽性細胞がなければホルモン受容体陰性(内分泌療法の適応なし),ER,PgR少なくともどちらか一方に陽性細胞が少数でも認められる場合には内分泌療法を考慮することは可能だが,占有率の低い症例(例えば10%未満)にはリスクとベネフィットのバランスに基づき内分泌療法適応の可否を決定する14)というのが現実的と考えられる。

個別化治療が進む中,症例個々に応じた柔軟な対応が求められる時代となっている。日本人乳癌におけるホルモン受容体陽性癌の増加が報告されているが21),将来に向けたさらなる治療の適正化という観点からも,詳細な情報を残しておくことが望ましい。病理としては,一定のカットオフ値を設け陰陽判定するよりも,陽性細胞占有率(あるいは染色強度も加味したAllred score)を情報として臨床に提供し,臨床はその他の臨床病理学的情報を総合的に勘案のうえ,対応を決定するのが当面望ましいと考えられる。
