・スタチンの服用が乳癌発症リスクを減少させるかどうかは結論付けられない。

背 景
スタチンは高LDLコレステロール血症の第一選択薬であり，コレステロール合成の律速酵素であるHMG—CoA還元酵素を拮抗的に阻害することで，強力なLDLコレステロール低下作用を示す。過去の臨床試験から，スタチンの虚血性心疾患や脳卒中の予防効果，総死亡率低減化が示されている。一方，培養癌細胞や類を用いた基礎的研究から，スタチンの発癌予防や抗癌作用が示されており（二次資料①②），これまでにさまざまな癌腫でスタチン服用と癌発症リスクとの関連性が検討されている。
解 説
過去に報告された症例対照研究9件1）～9）では，7つの研究においてスタチン服用による乳癌発症リスクの減少は認められなかった。2つの研究において，脂溶性スタチンであるフルバスタチン服用者のうち，服用期間5年以内の女性においてのみ，OR 0.5（95％CI 0.3-0.9）5），乳癌診断前3年以内に脂溶性スタチンであるロバスタチンを服用していた者においてOR 0.596（95％CI 0.497-0.714）とリスク減少が認められた9）。
コホート研究13件10）～22）のうち，乳癌発症リスクとの有意な関連性が認められたのは1件のみであった。2003年に報告された7,528人の高齢者を対象とした小規模コホート研究では，スタチン服用者の相対リスク（RR）は0.28（95％CI 0.09-0.86）と有意なリスク減少を認めている11）。Women’s Health Initiativeは154,587人の閉経後女性を対象とした大規模コホート研究であり，平均6.7年の経過観察時点では，脂溶性スタチン（シバスタチン，ロバスタチン，フルバスタチン）服用者において乳癌発症リスクの減少効果が示唆されたが14），その後の経過観察の結果，スタチンの服用と乳癌発症リスクとには有意な関連性が認められていない21）。
2005～2008年にかけて相次いでスタチンのランダム化比較試験を含めたメタアナリシスが報告されたが，いずれも乳癌発症リスクの減少効果は認められていない23）～25）。しかし，これらのメタアナリシスに含まれたスタチンに関する多くのランダム化比較試験は，心血管イベントを主要評価項目としているため，癌罹患の正確な情報が得られておらず，観察期間も短いとの指摘がある（二次資料③）。その後の，2008年に報告された12の症例対照研究を対象としたメタアナリシス26），2012年の13の観察研究と11の症例対照研究を対象としたメタアナリシスにおいても27），スタチンの服用と乳癌発症リスクには有意な関連性は認められていない。
以上より，これまでの大多数の研究で，スタチンの服用と乳癌発症リスクとの有意な因果関係を認めておらず，スタチンの服用が乳癌発症リスクを減少させるかどうかは結論付けられない。
検索キーワード
PubMedで“Breast Neoplasms”，“Hydroxymethylglutaryl—CoA Reductase Inhibitors”，“statin”のキーワードと同義語で検索した。医中誌・Cochrane Libraryも同等のキーワードで検索した。検索期間は2014年10月～2016年9月とし，76件がヒットした。該当文献から2件を選択し，2015年版で引用した25件に加え，合計27件の研究報告を解説に引用した。
