4．乳癌発症リスク―③合併疾患・治療薬

●スタチンの服用が乳癌発症リスクを減少させない可能性がある。

背　景

　スタチンは高LDLコレステロール血症の第一選択薬であり，コレステロール合成の律速酵素であるHMG-CoA還元酵素を拮抗的に阻害することで，強力なLDLコレステロール低下作用を示す。過去の臨床試験から，スタチンの虚血性心疾患や脳卒中の予防効果，総死亡率低減化が示されている。一方，培養癌細胞や齧(げっ)歯(し)類を用いた基礎的研究から，スタチンの発癌予防や抗癌作用が示されており（二次資料①②），これまでにさまざまな癌種でスタチン服用と癌発症リスクとの関連性が検討されている。

解　説

　過去に報告された症例対照研究10件1）～10）では，8つの研究においてスタチン服用による乳癌発症リスクの減少は認められなかった。2つの研究において，脂溶性スタチンであるフルバスタチン服用者のうち，服用期間5年以内の女性においてのみ，オッズ比（OR）が0.5（95％CI 0.3-0.9）5），乳癌診断前3年以内に脂溶性スタチンであるロバスタチンを服用していた者においてORが0.596（95％CI 0.497-0.714）とリスク減少が認められた9）。

　コホート研究13件11）～23）のうち，乳癌発症リスクとの有意な関連性が認められたのは1件のみであった。2003年に報告された7,528人の高齢者を対象とした小規模コホート研究では，スタチン服用者の相対リスク（RR）は0.28（95％CI 0.09-0.86）と有意なリスク減少を認めている12）。Women’s Health Initiative（WHI）は154,587人の閉経後女性を対象とした大規模コホート研究であり，平均6.7年の経過観察時点では，脂溶性スタチン（シバスタチン，ロバスタチン，フルバスタチン）服用者において乳癌発症リスクの減少効果が示唆されたが15），その後の経過観察の結果，スタチンの服用と乳癌発症リスクとには有意な関連性が認められていない22）。

　2005～2008年にかけて相次いでスタチンのランダム化比較試験を含めたメタアナリシスが報告されたが，いずれも乳癌発症リスクの減少効果は認められていない24）～26）。しかし，これらのメタアナリシスに含まれたスタチンに関する多くのランダム化比較試験は，心血管イベントを主要評価項目としているため，癌罹患の正確な情報が得られておらず，観察期間も短いとの指摘がある（二次資料③）。その後，2008年に報告された12の症例対照研究を対象としたメタアナリシス27），2012年の13の観察研究と11の症例対照研究を対象としたメタアナリシス28），2017年の20の観察研究と16の症例対照研究を対象としたメタアナリシスにおいても29），スタチンの服用と乳癌発症リスクには有意な関連性は認められていない。

　以上より，これまでの大多数の研究で，スタチンの服用と乳癌発症リスクとの有意な因果関係を認めておらず，スタチンの服用が乳癌発症リスクを減少させない可能性がある。

検索キーワード

　PubMedで“Breast Neoplasms”，“Hydroxymethylglutaryl-CoA Reductase Inhibitors”，“statin”，“Risk”のキーワードで検索した。検索期間は2014年9月1日から2016年9月30日までとした。該当37件の論文タイトルと抄録で2件を選択し，2015年版で引用した25件に加え，合計27件の研究報告を解説に引用した。2022年版での検索期間は2016年10月1日から2021年3月31日までとした。該当97件の論文タイトルと抄録で2件を選択し10）29），合計29件の研究報告を解説に引用した。
